## What comes through, and what stays.

Example with a fictional patient, Alex Morgan. Your clinical and IT teams agree the real scope with us.

In this example, five parts of the EHR record are shared through the FHIR connection and appear on the patient's page in Rhazes:

- Problems: type 2 diabetes, hypertension, right knee pain
- Medicines: metformin 500 mg twice daily, amlodipine 5 mg once daily
- Allergies: penicillin (rash)
- Results: HbA1c 52 mmol/mol, June 2026. BP 138/84
- Letters: diabetes review, June 2026

Admin notes (billing and scheduling) stay in the EHR because they are outside the agreed scope.

A clinician then asks Rhazes to “Summarise the history before today’s review” and gets a short summary linked to the problems, results and letters it came from.
